Strafford Community Nurse Program

Providing free compassionate support, education, and guidance to residents of all ages, helping individuals and families navigate health, aging, and care giving challenges while connecting them to local resources.

The Strafford Community Nurse can help residents with:
  • coordination of care among multiple health care providers
  • teaching and assistance in following health care advice
  • routine medication reviews to reduce error
  • assess of safety in the home to reduce risks of falls and injuries​
  • assess of social circumstances that effect health and quality of life and connect with Community Resources Coordinator 
  • family support for issues around aging, chronic illness, or palliative care

The Strafford Community Resources Coordinator can help residents to:

  • arrange for transportation to appointments, events, church, meal programs, etc.
  • arrange help with household chores such as: cleaning, laundry, yard work, snow removal, small repairs, etc. 
  • identify resources for short-term help with paying bills​
  • overcome social barriers to health
  • navigate local and statewide support services


Any Strafford resident is eligible to receive these services. All services are confidential and free of charge. You can contact SCN for yourself, a family member or a neighbor. The nurse will follow up with the referral to confirm they would like to speak to or visit with the nurse.
