Community Nurse
The Community Nurse offers the following support:
- Support for aging in place, including following PT/OT exercise plans
- Help scheduling primary care and specialist appointments
- Explain medical information from hospitals or offices
- Coordinate care among multiple providers and advocate for clients
- Review and manage prescription and non-prescription medications
- Link residents with physical and mental health resources
- Support chronic illness care, including nutrition and exercise counseling
- Assess home safety to prevent falls and injuries
- Provide family guidance around aging, chronic illness, or palliative care
- Help interpret Medicare, Medicaid, and private insurance notices
- Conduct well person checks for residents with family living remotely
Community Resources Coordinator
The Community Resources Coordinator provides support and coordination for residents and families. She can:
- Arrange transportation for:
- Medical appointments
- Non-medical needs (groceries, hair appointments, church, local events, errands, etc.)
- Coordinate pickup of:
- Prescriptions
- Food shelf items
- Mail and store orders
- Identify short-term financial assistance resources
- Assist in finding caregivers (short- or long-term)
- Household Support
- Coordinate help with:
- House cleaning and laundry
- Yard work and snow removal
- Small carpentry or handyman repairs
- Short-term meal preparation
- Lifting or moving heavy objects
- Decluttering or organizing spaces (e.g., garage)
- Home modifications such as grab bars, better lighting, or non-slip flooring
