Your Strafford Community Nurse (SCN) will:
- provide assistance in one or more of the following ways:
- how to understand medical information provided in the hospital or medical office.
- how to make medical decisions. Helps residents to determine where and how to get help for acute and/or complex medical issues.
- coordination of care among multiple health care providers.
- medication review and management. Reviews and educates about all medications and non-prescription drugs, and ensures they are being taken properly.
- linkage with the health and mental health systems.
- referrals to community services. Community services include transportation, meals, Neighbors helping Neighbors.
- support for chronic illness care. Counseling on nutrition and exercise.
- assessment of safety in the home to reduce risks of falls and injuries.
- family support for issues around aging, chronic illness or end of life. Information about options for care including assisted living, palliative care, and hospice.
- well person checks on residents whose concerned families live remotely.

PLEASE NOTE: The Community Nurse does not provide hands-on physical care. If skilled physical care or daily living assistance is needed, the community nurse will help the family explore options with home care agencies such as VNA, hospice care, or individuals in the community. The Community Nurse does not answer emergency calls. The Strafford Fast Squad provides this service through the 911 dispatch.
